Services Provided By Angel Watch

Assessment / Consultation
The initial client assessment will be lengthy. It includes a verbal client history, brief physical exam, review of current status i.e., medications / treatment, evaluation of personal milieu / support system. This assessment will be available to the client whether in a fairly autonomous setting or in a facility providing a higher level of care. A review of all information and recommendations will be covered during consultation. This assessment will become the basic building block of the clients' personal care plan.


Care Management
Following the assessment, a personal care plan that defines the extent of care management recommended will be designed. For example; a client may live in his/her own home, apartment or elderly housing. The client is becoming more frail, the family does not live in proximity and is concerned. Management of care may be accomplished by weekly or monthly visits and remain at a minimum - or alternative placement may be required, which Angel Watch would coordinate and monitor.


Coordination Support / Ancillary Services
The client may already have in place a support group of ancillary services i.e., meals on wheels, weekly cleaning or monthly visits by visiting nurse agencies, yet the family does not feel comfortable with the situation. Angel Watch will coordinate and monitor these services. There may also be situations involving multiple medical services, requiring Angel Watch to become the liaison withing the medical community. The truth is that many times one medical hand does not always know what another medical hand is doing.


Continuity of Information
One of the most important services provided by Angel Watch is the continuity of information relayed to the client and family. The availability of an ongoing progress report can be of great comfort - people want to know what is going on.


Alternative Care Placement
The reality of longevity is that sometimes an appropriate alternative to the present situation is required. This decision is a very difficult one for the supporting family. WHAT TO DO? WHERE TO GO? WHAT IS AVAILABLE? HOW MUCH WILL IT COST? WILL IT BE SATISFACTORY? Angel Watch will present it's recommendation, assist in the transition and continue to monitor the client.
